STUDENT REGISTRATION CONTRACT
ExpressLane Defensive Driving (CP 550), hereinafter referred to as "school," hereby agrees to provide
the undersigned parties, hereinafter referred to as "student", with six (6) hours of driving safety
instruction upon receipt of the entire cost of the course.
GENERAL INFORMATION: The school is prohibited from issuing a certificate of completion if a student has
not met all of the requirements for the course completion, and a student should not accept a certificate
under such circumstances. This agreement constitutes the entire contract between the school and a
student. No verbal assurances or promises not obtained herein shall bind the school or a student.
CANCELLATION POLICY: A refund will be made to any student who cancels the student registration contract
within 30 days after registering unless the student has successfully completed the course or received a
failing grade on the course examination. The school will charge an $8.00 processing fee for any refunds,
which will be removed from the course payment prior to refund.
REFUND POLICY: All tuition and fees paid are due and refundable when:
- The course of instruction is discontinued by the school, preventing a student from completing
the course.
- The enrollment of a student was procured as a result of any misrepresentation in advertising,
promotional materials of the school, or the result of any misrepresentation made by an owner
or employee of the school.
All refunds will be completed within 30 days after the effective date of enrollment termination.
ATTENDANCE POLICY: A student must attend the complete 6-hour course including all phases of the
approved school curriculum and pass the final exam in order to receive a certificate of completion. The
student who registers for the course must be the same person who attends the course. The school will
validate the student's identity throughout the course using timed personal validation questions. If the
student misses more than 30% of the personal validation questions they will be excluded from the
course and will not receive a completion certificate. The student has 120
days from their registration date to complete the course after which the student will be shown to have withdrawn from
the course which constitutes a failed attempt and therefore no refund will be granted.
GRADING AND PROGRESS POLICY: A student may not be certified or given credit for a driving safety course
unless the student scores 70% or higher on the final exam. Alternative testing techniques for students
with reading, hearing, or learning handicaps, and policies for retesting students who score less than 70%
will be as follows: A. Review of the material; B. Translation, if necessary; C. Retest, verbally, if necessary.
STUDENT ACKNOWLEDGMENTS: By agreeing to this form I signifies that I have been informed of the
cancelation and refund policy; and school regulations pertaining to absence, grading policy, progress,
and rules of operation and conduct. I realize that any grievance not resolved by the school and/or
